CJCSM 3150.03D is no longer current. The CJCS Directives system as of 2026 lists no 3150.03 successor. Marine Corps replacement

For OPREP-3 procedures applicable to Marine Corps units, use MCO 3504.2A - Operations Event/Incident Report (OPREP-3) Reporting. MCO 3504.2A implements OPREP-3 reporting in the Marine Corps chain and is current. It carries the OPREP-3 SIR (Serious Incident Report) format, PINNACLE thresholds, NMCC and MCOC contact information, and sister-Service variant cross-references in its enclosures.

CJCSM 3150.03D established the Joint Reporting Structure (JRS) requirements for event and incident reports, specifically the OPREP-3 reporting system. The manual defined flag word categories for OPREP-3 reports, established national-level interest thresholds triggering PINNACLE-level reports to the National Military Command Center (NMCC), and prescribed the serious incident report (SIR) criteria for significant but sub-PINNACLE events.

OPREP-3 reports fell into two primary categories:

• PINNACLE. National-level interest events requiring immediate report to the NMCC within 15 minutes of commander awareness. Flag words include NUCFLASH, BROKEN ARROW, BENT SPEAR, DULL SWORD, EMPTY QUIVER, FADED GIANT, BLEEDING EDGE, LAME DUCK, HELPING HAND, and others tied to specific event types.
• SIR (Serious Incident Report). Significant events below the national-level interest threshold. SIRs route through the operational chain of command. When in doubt whether a threshold is met, commanders escalate to PINNACLE rather than SIR.
