DD Form 2675 is the claim form for adoption expense reimbursement under 10 U.S.C. 1052 and DOWI 1341.09. Complete one form for each adopted child.

How the Marine Corps routes it

Under MARADMIN 501/26, the Marine submits the form and supporting documents through the chain of command to the servicing Personnel Administrative Office. The PAO verifies eligibility and forwards the package with a NAVMC 11116 to the disbursing or finance office in DTMS.
