DD Form 2704 - Victim and Witness Certification and Election Concerning Prisoner Status

Scope

DD Form 2704 records the victim or witness election on post-trial notification of prisoner status changes. The form captures the victim or witness identifying information, the case data (incident number, installation, court-martial caption, accused name, last four of SSN), and the elections governing notice of confinement status changes, parole, release, transfer, and escape. Prescribed by DoDI 1325.07. Updated 19 March 2025. CUI when filled in.
