MARADMIN 754/20 - COVID-19 Vaccine Guidance for Active and Reserve Components

Scope

MARADMIN 754/20 sets initial Marine Corps guidance for active and reserve components to implement the DoD COVID-19 Vaccination Plan in support of Operation Warp Speed (OWS) and the whole-of-government response to the SARS-CoV-2 pandemic. Vaccine offering, prioritization, distribution, execution, reporting, and Force Health Protection (FHP) posture are addressed. The source page reads CANCELLED. Audience

• HQMC Health Services. Owns vaccine reporting and policy coordination.
• HQMC PP&O COVID-19 Cell. Operational center coordinating distribution and reporting.
• Marine Corps Operations Center (MCOC). Receives OPREP-3 CCIR reports on vaccine adverse events, post-vaccination COVID-19 infections, and public interest.
• Military Treatment Facility (MTF) leaders. Execute and administer COVID-19 vaccination.
• Defense Health Agency (DHA). Tasked with executing the DoD COVID-19 Vaccination Plan.
• BUMED and NAVMEDLOGCOM. Vaccine allocation chain.
• Commanding officers. Coordinate with MTFs. Confirm 100 percent of personnel are offered vaccination. Document MRRS entries. Plan staggered vaccination evolutions for operational readiness.
• Healthcare providers (HCPs). Counsel on risks, benefits, alternatives. Screen with DHA Form 207. Document the EHR and MRRS.
• Marine Corps active duty, reserve component, civilian employees, dependents, retirees, eligible DoD beneficiaries.

Authorization and eligibility

• Authorization under Emergency Use Authorization (EUA). Vaccination under EUA is not mandated.
• Eligible. Service members on active duty and in the Selected Reserve. Dependents, retirees, and DoD beneficiaries. DoD civilian employees not otherwise eligible DoD beneficiaries. Select contractor personnel routinely receiving influenza vaccines as part of a DoD occupational safety and health program. Select Other Than US Forces personnel approved case-by-case at DoD level.
• Service members and dependents pull vaccine at MTFs, approved DoD vaccination sites, or TRICARE-Network providers and pharmacies when available and approved. DoD civilians and select contractor personnel pull only from DoD MTFs or approved DoD vaccination sites.

Phased prioritization

1. Phase 1a. Healthcare workers, first responders, public safety personnel.
2. Phase 1b.1. Critical national capabilities. Strategic and National mission forces.
3. Phase 1b.2. Personnel preparing to deploy OCONUS.
4. Phase 1b.3. Other critical and essential support personnel. Most Marine Corps service members.
5. Phase 2. Beneficiaries at high risk of severe COVID-19 disease per CDC definition.
6. Phase 3. Healthy personnel and other beneficiaries.

Distribution phases

• Phase 1. Pre-Vaccination. Planning. Ended with the first COVID-19 vaccine FDA EUA.
• Phase 2a. Controlled Pilot and Limited Distribution. Began at the FDA EUA. Twelve CONUS primary sites, four OCONUS primary sites, six secondary sites. Ends after DHA validates the controlled pilot.
• Phase 2b. Expanded Distribution. Vaccine ordering transitions to the Services. Ends at 60 percent DoD COVID-19 vaccine requirement met.
• Phase 2c. Saturation. Routine vaccination ordering and distribution.

Execution highlights

• Vaccine administration runs centrally through MTFs and other approved DoD vaccination sites at release.
• Service members preparing to deploy receive the first dose of any two-dose vaccine no later than 60 days before deployment to allow time for the second dose and side-effect monitoring.
• Commanders apply staggered vaccination schedules for critical personnel to preserve operational readiness during the 12 to 72 hour post-vaccination side-effect window.
• Personnel in Flight or Dive status are grounded or excluded from high-risk activities for 72 hours after mRNA vaccination. 12 hours after any other COVID-19 vaccine.
• Documentation. EHR captures vaccine name or CVX code, manufacturer, lot number, dose, route, anatomic site, and HCP name. Or exemption codes medical declined or medical temporary.
• For active duty, the EHR information also enters Medical Readiness Reporting System (MRRS).
• Personnel vaccinated at approved non-DoD facilities (example, civilian pharmacies) submit documentation to the command MRRS representative no later than 1600 on the next regular duty day.

Reporting

CCIRs under this MARADMIN.

• Adverse reaction to a dose.
• COVID-19 infection post-vaccination.
• Negative press, social media, or other public interest in the Marine Corps or DON COVID-19 vaccination process.

Reports submit via OPREP-3 to HQMC PP&O Marine Corps Operations Center, operational chain of command, and administrative chain of command. MCOC, HQMC PP&O COVID-19 Cell, and Health Services then report to the Joint Staff and courtesy copy DHA Operations as appropriate.

Adverse Events. HCPs administering COVID-19 vaccine report to the Vaccine Adverse Event Reporting System (VAERS) at https://vaers.hhs.gov/reportevent.html per legal requirement.

Force Health Protection posture

• COVID-19 FHP posture stays unaltered, individually or Force-wide, regardless of vaccination status or command vaccine coverage.
• It is unknown whether the released vaccines prevent asymptomatic SARS-CoV-2 transmission.
• It is unknown how long vaccine protection lasts, whether boosters are required, and at what cadence.
• Continued compliance with COVID-19 FHP requirements expected for the foreseeable future.
