Scope
NAVMC 11533, Personal Award Recommendation, revision 12-12, electronic fillable (EF). Three pages: a Privacy Act statement, the recommendation with items 1 through 22, and the endorsement and disposition page with items 23 and 24. MMMA built the fillable PDF to mirror the iAPS web based view, so the paper form and the iAPS case carry the same fields. The form is FOUO and privacy sensitive when filled in. It is the standard template for Combat Action Ribbon nominations and the hardcopy path for personal award recommendations originating outside iAPS, for example from joint commands.

Records a nomination for a Department of the Navy personal award. Privacy Act authorities are 10 U.S.C. 5013 and 5041, SECNAVINST 1650.1H, and E.O. 9397. Either the SSN or the EDIPI is accepted by MMMA for nominee identification.

- Nominee identification. EDIPI or SSN, designator or NEC or MOS, name, component, grade, warfare designator for Navy nominees, and RUC.
- The recommendation. Recommended award, whether the nomination covers a specific achievement, and the type: Heroic, Meritorious, Heroic Posthumous, Meritorious Posthumous, or MIA. Number of the award of the recommended medal, action date or meritorious period, and geographic area of action or service.
- Timing drivers. Expiration of active duty, estimated detachment date with the reason marked Retirement, Transfer, Terminal Leave, or Impact Award, years of service when retirement applies, and the new duty station or home address when separation is anticipated.
- Context for the awarding authority. Unit at the time of action or service, duty assignment, previous personal decorations with periods recognized, personal awards recommended and not yet approved, and other personnel being recommended for the same action.
- Certification. The originator certifies the facts in the summary of action are known to them or a matter of record, with name, grade, title, signature, date, command POC, and phone number.
- The form directs submission of the Summary of Action and citation, when applicable, in a separate Microsoft Word document.
Routing and disposition
Item 23 records forwarding endorsements by each via addressee, including the recommended award and Combat V position of every endorsing command, with signature, grade, and date forwarded. Item 24 belongs to the awarding authority: disposition of the basic recommendation, whether extraordinary heroism is recommended, signature with grade and title, and date approved. For Marines retiring from joint commands, the PCS season MARADMINs route the completed form through the first General or Flag Officer in the joint chain to the MMPB-3 shared mailbox.
