Are appeals requesting stays of punishment involving restriction/extra duties acted on within five days? If not are punishments stayed?

Are all the appropriate blocks of each UPB for the past 3 calendar years filled out in accordance with the reference? Item 1: Does this include article(s) of the UCMJ violated and a summary of each offense alleged to include date, time, and place of the alleged offense. Item 2: Requires advising the accused in accordance with Article 31, UCMJ. Did the accused indicate intentions by selecting from drop-down choices or by manually writing in their intentions? Did the accused date and sign? If accused refused to sign, is the “refused to sign” box checked and did the CO sign in this block instead? Item 3: Completed upon ensuring that the individual has been informed of rights enumerated in item 2. Did the immediate commanding officer date and sign item 3 prior to imposition of NJP. Item 4: Does this block contain current and previous UAs in excess of 24 hours and current and previous desertions recorded? Item 5: Is the correct drop down selected for each charge or written in for each charge? Item 6: Record complete and accurate punishment(s) imposed followed by date of imposition. Item 7: Record date of suspension followed by complete and accurate description of all or parts of punishment being suspended. If none, is the word "NONE" recorded? Item 8, 8A, and 8B: Is complete name, rank/grade, title, and EDIPI of the officer imposing punishment listed? (i) Item 9: Is this block signed by the officer imposing punishment? (j) Item 10: Does this block properly reflect the date of notification? (k) Item 11: Is this block signed and dated by the officer imposing punishment? (l) Item 12: Did the accused select a choice from the drop-down selections and signe? (m) Item 13: Is this block dated? If not appealed, is the “Not appealed” box checked? (n) Item 14: Has the officer who acted on the appeal completed this block or has it been left blank (no appeal).
