Purpose
The Unit Punishment Book (UPB), NAVMC 10132NAVMC 10132, is the legal document of record for Non-Judicial Punishment proceedings under Article 15 UCMJ. S-1 admin clerks prepare the UPB for the commander before mast and document results after. The UPB drives unit diary entries, OMPF filing, and pay impacts.
When You Prepare a UPB
- Commander has accepted an NJP referral.
- Member has elected to accept NJP (or refused trial by court-martial).
- Charges have been documented and witness statements collected.
- The mast is scheduled and on the calendar.
Conditions and Standard
Per T&R event 0111-GENA-1003, you perform this work with access to web-based systems (Command Legal Action System CLA) or database systems, the Electronic Service Record (ESR), and source documents on hand. The standard is to ensure no format, typographical, or grammatical errors.
Performance Steps
Pre-Mast (Blocks 1 through 5 and 18 through 21)
- Block 1 - Member identification. Pull from MCTFS. Verify SSN, full name, rank, MOS, unit. Cross-check against ID card if available.
- Block 2 - Charges. List each specification under the relevant UCMJ article. Use the language from the Manual for Courts-Martial. Do not paraphrase.
- Block 3 - Date and place of offense. Extract from the witness statements and command investigation. Be precise.
- Block 4 - Notification. Document when the member was notified of NJP referral. Member signs acknowledgment.
- Block 5 - Election. Member elects acceptance of NJP or demand for trial by court-martial. Witness signature required.
- Block 18 through 21 - Procedural acknowledgments. Member rights advisement, opportunity to consult counsel, opportunity to present matters in extenuation and mitigation.
Post-Mast (Blocks 6 through 17)
- Block 6 - Findings. Commander enters guilty or not guilty for each specification.
- Block 7 through 12 - Punishment. Document each punishment imposed. Common entries include reduction in grade, forfeiture of pay, restriction, extra duties, correctional custody.
- Block 13 through 15 - Suspended punishments. Note any suspended terms and the suspension period.
- Block 16 - Appeal rights. Member acknowledges right to appeal. Member elects to appeal or waive.
- Block 17 - Commander signature. Imposing commander signs. Date matches the mast date.
Required Attachments
Per MCO P5800.16MCO P5800.16 LSAM, the UPB packet includes the following.
- Charge sheet with specifications
- Investigation summary or command investigation
- Witness statements (signed and witnessed)
- Member acknowledgment of rights
- Counseling Page 11 entries leading up to the offense
- Prior NJP records on file
- ESR printout showing service history
Pay Impact Coordination
Forfeitures and reductions affect pay. Coordinate with the supporting PAC.

Common Errors
S-1 audits surface these errors most often on UPB prep.
- Incomplete charge specification. Specification language deviates from MCM. Fix by using MCM verbatim.
- Missing rights advisement signature. Member did not sign Block 18 acknowledgment. Fix before mast.
- Wrong commanding officer signature. Acting commander not properly designated in writing. Fix by attaching designation memo.
- Late forfeiture transaction. UD/MIPS not submitted within the pay period. Fix by submitting with retroactive effective date.
- Page 11 entry not posted. Required Page 11 documenting NJP did not make it into the SRB. Fix immediately and route to OMPF.
