What this form is
Form 137, Secondary Dependency Application. Fillable PDF on file from the 2026-04-29 forms archive snapshot. The form recognizes a non-spouse, non-biological-child dependent for BAH, USIP card, and travel allowance purposes. The OCT 2024 version covers seven dependency types. Incapacitated adult child, student age 21 to 22, legal custody ward, parent, parent-in-law, stepparent, and in-loco parentis. The over-half financial support test applies to every type. The Worksheet for Determining Financial Support sits on pages 5 to 7 and substitutes for a prior-year tax return showing the claimed individual as a dependent.

Who prepares, signs, and adjudicates
The Marine sponsor completes sections 1 through 8 and signs section 9, with the claimed individual or a representative signing where required. S-1 reviews the package for completeness and routes it to IPAC. IPAC routes it to the HQMC Dependency Determination Section for adjudication. In-loco parentis claims add DFAS Form 9124 and two DFAS Form 9125 third-party affidavits from non-relatives.
What approval drives
Approval triggers DEERS enrollment, a BAH adjustment through MCTFS, and a RAPIDS appointment for the USIP card where residence rules allow. Parents, parents-in-law, and stepparents need residence with the sponsor for the USIP card. In-loco parentis dependents never receive one. BAH and travel allowance carry no residence requirement for those types. Every approved case recertifies annually per MARADMIN 661/19 with an updated form and Worksheet. The filled-in form is CUI under the PRVCY category. Handle it accordingly and keep it off unencrypted external email.

The governing authorities are DoD 7000.14-R Volume 7A Chapter 26 and MCO 1751.3. MCTFSPRIUM 12-0103 directs units to submit DD Form 137-6 to CMC (MFP-1) to retain a dependent child past age 21.
