DD Form 2701 - Initial Information for Victims and Witnesses of Crime

Scope

DD Form 2701 delivers initial victim and witness rights information at the start of a military criminal investigation or prosecution. The form lists the statutory rights of a crime victim, identifies the points of contact at the command, legal office, and military attorney level, and captures the victim's election regarding legal representation. Prescribed by DoDI 1325.07. Updated 21 August 2024.

Audience

• Special Victims' Counsel (SVC) and Victims' Legal Counsel (VLC). Provide DD 2701 to eligible victims at first contact.
• Victim Advocates and Sexual Assault Response Coordinators (SARC).
• Family Advocacy Program (FAP) staff.
• Military criminal investigators. NCIS, CID, OSI, or service-equivalent.
• Trial counsel and Staff Judge Advocate offices.
• Command staff. Receive copies for the case file.
