PAA 12-12 - Mailing Address for Copy 5 of DD Forms 214 and 215 to the Department of Labor

Scope

PAA 12-12 corrects the mailing address for Copy 5 of DD Forms 214 and 215 sent to the Department of Labor. A DoD review of separation procedures found inconsistent mailing of Copy 5 to the correct address. The advisory directs all separation processors to use the Federal Claims Control Center (FCCC) address and to update desk top procedures. Effective date 2012-09-25.
