Reportable Events and Incidents (Enclosure 2)

Role and responsibility

The unit watch officer, operations duty officer, and S-3 staff identify reportable events against the Enclosure 2 list. The commander makes the reportability assessment for events on the threshold. The S-1 officer and adjutant maintain the unit reportable events checklist as a duty-officer reference.

Enclosure 2 is a general guideline and is not all-inclusive. The MCO 3504.2A standing rule is "when in doubt, submit a report." Train duty officers and watch officers on the list and the standing rule.

The reportable events categories

Enclosure 2 of MCO 3504.2A groups reportable events into broad categories. The actual Enclosure 2 list is the authoritative source. The categories below frame the scope.

Death, serious injury, and medical events

• Death of a Marine, Sailor, or DoD civilian in any circumstance (combat, training, mishap, suicide, natural causes, etc.).
• Serious injury requiring hospitalization.
• Suicide attempt resulting in serious injury or hospitalization.
• Medical evacuation (MEDEVAC) from a deployed location.
• Significant medical emergency at the unit.

Criminal and disciplinary events

• Sexual assault (route per SAPR confidentiality rules in the sexual-assault-and-hazing-report-protocol sub-page).
• Hazing event (route per Enclosure 6 sample).
• Significant UCMJ violation including court-martial referral.
• Drug-related event including positive urinalysis above the unit reporting threshold.
• Theft or unauthorized disposition of military property above the reporting threshold.
• Domestic abuse or child abuse per MCO 1700.28A Family Advocacy Program.

Mishap events

• Aviation mishap per MCO 3040.4 (coordinate with the safety board).
• Ground mishap involving fatality, serious injury, or significant equipment damage.
• Weapons mishap, ammunition mishap, or explosives mishap.
• Things Falling Off Aircraft (TFOA) (notify RTAMB, Range and Training Area Management Branch).

Operational events

• Significant operational incident below the national-level interest threshold.
• Combat operation event resulting in casualty.
• Loss of communications with a deployed unit.
• Significant equipment loss in operational context.

Force protection events

• Active shooter event at a Marine Corps installation.
• Bomb threat at a Marine Corps installation above the response threshold.
• Significant force protection incident.

Public affairs and political events

• Event likely to generate national media attention.
• Significant adverse public affairs event.
• Civil disturbance event involving Marine Corps personnel or installations.

Processing workflow

1. Maintain the duty officer reference. The S-1 officer maintains a unit-tailored reportable events checklist on the duty officer desk, referencing Enclosure 2 of MCO 3504.2A.
2. Train duty officers and watch officers. Train new duty officers on the Enclosure 2 list and the "when in doubt, submit" framework during the duty officer indoctrination.
3. Receive the event notification. The duty officer or watch officer receives the event notification from the chain of command, the medical channel, the law enforcement channel, or another source.
4. Match the event against Enclosure 2. The duty officer cross-references the event against the Enclosure 2 list. Match against specific categories (death, sexual assault, hazing, mishap, etc.) per the unit reference card.
5. Apply the "when in doubt, submit" rule. For events not clearly on Enclosure 2, the duty officer escalates to the commander. The default is to submit per MCO 3504.2AMCO 3504.2A Paragraph 3c(3)(a).
6. Assess national-level interest. The commander assesses against the PINNACLE threshold per the oprep-3-pinnacle-and-national-level-interest-reporting sub-page. Non-PINNACLE events route as SIR.
7. Document the reportability assessment. The duty officer records the reportability decision in the duty log, including the event description, the Enclosure 2 match (or non-match), and the commander assessment.
8. Submit the OPREP-3 SIR. Route per the oprep-3-sir-serious-incident-reporting sub-page workflow.
9. Coordinate with adjacent programs. Sexual assault routes through the SARC per MCO 1752.5A. Mishap coordinates with the safety board per MCO 3040.4. Domestic and child abuse coordinates with FAP per MCO 1700.28A.
10. Update the unit reportable events checklist. The S-1 officer refreshes the unit checklist after each OPREP-3 SIR submission, capturing lessons learned and clarifying borderline event handling.

Common pitfalls

• Enclosure 2 treated as exhaustive. The duty officer rules out reportability because the event is not literally on the list. Apply the "when in doubt, submit" rule per MCO 3504.2AMCO 3504.2A Paragraph 3c(3)(a).
• Duty officer indoctrination skipped on the Enclosure 2 list. The duty officer reports late or fails to report. Train every new duty officer on the Enclosure 2 framework.
• Adjacent program coordination skipped. Sexual assault reports without SARC coordination, mishaps without safety board coordination, or domestic abuse without FAP coordination. Coordinate at the moment of event awareness.
• TFOA notification to RTAMB skipped. The Things Falling Off Aircraft report routes only to MCOC without notifying Range and Training Area Management Branch per MCO 3504.2AMCO 3504.2A Paragraph 5c.
• Unit reportable events checklist stale. The duty officer reference lags policy refresh. Update the checklist on each MCO 3504.2A update and after lessons-learned from prior SIR submissions.

Decision points

• Reportable versus not reportable. The default under MCO 3504.2AMCO 3504.2A is to submit when in doubt. The commander makes the call on borderline events.
• National-level interest versus SIR. The commander assesses PINNACLE threshold per MCO 3504.2A. Significant events with broad strategic implications escalate to PINNACLE.
• Adjacent program routing. Sexual assault routes through SARC. Mishap routes through safety board. Domestic and child abuse routes through FAP. Route in parallel with the OPREP-3 SIR submission.
