Role and responsibility
The S-1 officer and the SARC coordinate the OPREP-3 SIR sexual assault report content to apply the Paragraph 5a confidentiality rules. The watch officer or duty officer drafts the initial voice report with rank or grade only. The S-3 operations staff drafts the asynchronous message report per Enclosure 7 sample format. The commander signs the report. The hazing report runs in parallel under MCO 1700.28 with the Enclosure 6 sample format.

Sexual assault report confidentiality is a statutory and policy requirement. Failure to apply the Paragraph 5a rule exposes the victim or witness to confidentiality compromise and violates MCO 1752.5A SAPR Program.

The Paragraph 5a confidentiality protocol
MCO 3504.2A Paragraph 5a establishes the sexual assault OPREP-3 SIR confidentiality protocol. The protocol applies to victim, witness, and suspect identification in the report:
- Provide only rank or grade. Do not provide initials, last name, or last four of SSN.
- When the rank or grade would compromise confidentiality (e.g., the only Marine of the rank in the unit), state "omitted to protect confidentiality" under the rank field.
- Refer to Enclosure 7 for the sample sexual assault OPREP-3 SIR message format.
For non-sexual-assault reports, include only the EDIPI for personnel identification per Paragraph 5b and DODI 1000.30. Do not include SSN or last four of SSN on any OPREP-3 report.
The Enclosure 6 hazing protocol
MCO 3504.2A Enclosure 6 provides the sample hazing OPREP-3 SIR message format. Hazing reports run in parallel with the Marine Corps Hazing Prevention Program under MCO 1700.28. The OPREP-3 SIR hazing report includes the EDIPI for personnel identification per Paragraph 5b (the Paragraph 5a confidentiality rule applies to sexual assault, not hazing).
Processing workflow
- Receive the event notification. The duty officer or SARC receives the sexual assault notification (Unrestricted Report) or the hazing complaint.
- Verify the OPREP-3 SIR trigger. Restricted sexual assault reports under MCO 1752.5A do not trigger OPREP-3 SIR submission. Verify the report status with the SARC before the OPREP-3 SIR submission.
- Coordinate with SARC (sexual assault) or the hazing prevention coordinator (hazing). The SARC provides case factual content within confidentiality limits. The hazing prevention coordinator provides hazing case context.
- Submit the synchronous voice report. The watch officer calls MCOC within 30 minutes per MCO 3504.2AMCO 3504.2A Paragraph 4a(2)(a)2 with rank or grade only for sexual assault cases, or with EDIPI for hazing cases.
- Draft the OPREP-3 SIR message. Use the Enclosure 7 sample for sexual assault cases or the Enclosure 6 sample for hazing cases.
- Apply the Paragraph 5a confidentiality rule for sexual assault. Provide only rank or grade for victim, witness, and suspect. Omit initials, last name, and last four of SSN. State "omitted to protect confidentiality" where rank or grade compromises confidentiality.
- Apply the EDIPI rule for hazing. Include only the EDIPI per Paragraph 5b. Do not include SSN or last four.
- Route the message report. Submit to MCOC per Enclosure 3 message addressing. Include CMC WASHINGTON DC PPO as required addressee.
- Submit supplemental reports. Update facts as the investigation develops. Apply the same confidentiality rule on every supplemental report.
- File the OPREP-3 case file. Place the voice report log, the message report, the supplemental reports, and the parallel SARC or hazing prevention case file cross-reference in the unit OPREP-3 case file.

Common pitfalls
- Sexual assault report includes prohibited PII. The OPREP-3 SIR exposes the victim or witness through initials, last name, or last four of SSN. Apply the rank-or-grade-only rule per Paragraph 5a and Enclosure 7.
- Rank-or-grade-only rule applied without confidentiality omission. The victim is the only Marine of the rank in the unit and the rank itself identifies the victim. State "omitted to protect confidentiality" per Paragraph 5a.
- Restricted Report converted to OPREP-3 SIR without victim election. Restricted Reports under MCO 1752.5A do not trigger OPREP-3 SIR submission. Verify the report status with the SARC before submission.
- Hazing report uses the Enclosure 7 sexual assault template. The hazing report uses the Enclosure 6 hazing sample format with the EDIPI rule per Paragraph 5b.
- SARC or hazing prevention coordination skipped. The OPREP-3 SIR routes without parallel program coordination. Coordinate with the SARC for sexual assault and the hazing prevention coordinator for hazing.
