Personnel Casualty Report (PCR) Processing - S-1 Procedural Page

Role and responsibility

The S-1 chief or designated casualty reporting clerk prepares and submits the PCR within the timeline per MCO 3040.4MCO 3040.4 Chapter 3. The commander authorizes PCR submission. The S-1 officer signs by direction on PCR routing. HQMC Casualty Section receives the PCR and coordinates national-level reporting.

Source documents flow from the incident report, NCIS or law enforcement notifications, hospital reports, the Marine's MCTFS record, and the DD Form 93 emergency data card. PCR submissions route through the casualty reporting chain to HQMC Casualty Section. Outputs include the certified PCR, the MCTFS casualty status update, the DD Form 1300 Report of Casualty per Chapter 3 Section 2, and the casualty case file foundation.

Per MCO 3040.4MCO 3040.4 Chapter 3 Section 1, reportable casualties include Marines and other persons defined in Sections 1 and 2. Nonreportable casualties at Section 3 fall outside PCR processing. Trigger events

• Reportable Marine casualty incident per Chapter 3 Section 1.
• Reportable casualty of other person per Chapter 3 Section 2.
• Initial casualty notification from NCIS, hospital, or chain of command.
• Status change requiring follow-up PCR (Marine moved from hospitalized to deceased, missing to deceased).
• Supplemental PCR triggered by new information.
• DD Form 1300 preparation triggered by confirmed death per Chapter 3 Section 2 paragraph 2.
• Mass casualty event triggers multiple PCRs.

Processing workflow

1. Identify the reportable casualty. Validate against Chapter 3 Section 1 (Marine reportable) or Section 2 (Other Reportable). Nonreportable casualties (Section 3) drop from PCR processing.
2. Determine the PCR type. Per MCO 3040.4MCO 3040.4 Chapter 3 Section 2 paragraph 1 (PCR Requirements), select the right PCR type for the incident.
3. Prepare PCR content. Pull EDIPI, rank, unit, dependency state from MCTFS. Capture incident facts. Per MCO 3040.4MCO 3040.4 Chapter 3 Section 2 paragraph 1, follow PCR content requirements.
4. Validate categorization. Hostile, non-hostile, training-related, or off-duty drive notification routing and benefit processing.
5. Submit PCR. Route through the casualty reporting chain to HQMC Casualty Section. The S-1 chief submits.
6. Cut MCTFS casualty status code. Submit through UD-MIPS per MCTFSPRIUMMCTFSPRIUM casualty transaction format.
7. Distribute the PCR. Per MCO 3040.4MCO 3040.4 Chapter 3 Section 2 paragraph 3 (PCR Distribution), route copies to specified recipients.
8. Prepare DD Form 1300. Per MCO 3040.4MCO 3040.4 Chapter 3 Section 2 paragraph 2 (Reporting Requirements), prepare DD Form 1300 Report of Casualty when death is confirmed.
9. File the certified PCR. Stage in the casualty case file and route copies to OMPF Field Folder.
10. Coordinate with CACO. The CACO assignment triggers the PNOK notification chain per Chapter 4.

• Wrong casualty categorization. Hostile, non-hostile, training, or off-duty drive different routing. The S-1 officer validates before submission.
• PCR distribution incomplete. Per Chapter 3 Section 2 paragraph 3, distribution list is specific. Pull the list before submission.
• DD Form 1300 omitted. Per Chapter 3 Section 2 paragraph 2, the DD Form 1300 is required when death is confirmed.
• MCTFS casualty status code not updated. Pay continues incorrectly. Cut the status code at PCR submission per MCTFSPRIUMMCTFSPRIUM.

Decision points

• Reportable versus nonreportable. Per MCO 3040.4MCO 3040.4 Chapter 3 Sections 1, 2, and 3, the incident falls in or out of PCR scope. The S-1 chief validates.
• Initial versus supplemental PCR. New information arrives after initial PCR. Per Chapter 3 Section 2 paragraph 2, submit a supplemental PCR.
• Mass casualty PCR sequencing. Multiple casualties trigger multiple PCRs. The S-1 chief coordinates sequencing to keep PNOK notifications in proper order.
