TRICARE for Life (TFL) for Reserve Retirees
TRICARE for Life is the secondary payer to Medicare for retirees age 65 and over. Reserve retirees with Medicare Part A and B enrolled receive TFL automatically. No premiums, no enrollment fees beyond Medicare Part B premium.

What TFL Is
TRICARE for Life (TFL) is the wraparound coverage for Medicare-eligible military retirees. It is secondary payer to Medicare. Reserve retirees age 65 and over with Medicare Part A and B receive TFL automatically through their TRICARE retiree status.
I&I admin handles fewer TFL questions than TRS or TRR (since most Reserve retirees age 65+ live as veterans, not active reservists). When questions come up, knowing the basics helps. Drilling SELRES (Selected Reserve, what most Marines call SMCR or the Reserves) members sit under TRS, not TFL.
Eligibility
A Reserve retiree qualifies for TFL when meeting all of these.
- Reserve Retired status (drawing Reserve retirement pay or eligible to draw)
- Age 65 or older (or qualifying for Medicare due to disability)
- Enrolled in Medicare Part A (typically free, premium-free for most)
- Enrolled in Medicare Part B (monthly premium required)
- Enrolled in DEERS
The Medicare Part B premium is the only out-of-pocket coverage cost for most TFL beneficiaries.
How TFL Works
TFL pays after Medicare. The flow.
- Beneficiary receives medical service
- Provider bills Medicare first
- Medicare pays the Medicare allowable amount
- Provider bills TFL for the remaining cost
- TFL pays nearly all remaining costs (with some exceptions)
- Beneficiary typically pays little or nothing for TRICARE-covered services
This makes TFL essentially comprehensive coverage for Medicare-eligible retirees.
What TFL Covers
TFL covers what TRICARE covers, secondary to Medicare.
- Inpatient and outpatient services Medicare covers
- Mental health services
- Maternity care (rare for this age group)
- Preventive services
- Prescription drug coverage through TRICARE pharmacy benefits
Items Medicare does not cover but TRICARE does are paid by TFL as primary.
Costs to the Beneficiary
Out-of-pocket costs for TFL beneficiaries are limited.
- Medicare Part B premium (set annually by Medicare)
- Some non-covered services
- Costs above TRICARE allowable for non-network care
- Some prescription drug copays at retail pharmacy
For most TFL beneficiaries, total out-of-pocket beyond Medicare Part B premium is minimal.
Enrollment
Unlike TRS and TRR, TFL has no separate enrollment.

- Member must enroll in Medicare Part A and B (separate process with Social Security)
- DEERS must show retiree status with Medicare information
- Coverage activates automatically when DEERS reflects Medicare enrollment
I&I admin counsels retirees to coordinate with Social Security for Medicare enrollment.
Dependents
Eligible dependents also receive TFL when they meet the criteria.
- Spouse age 65 with Medicare Parts A and B
- Spouse with disability qualifying for Medicare
- Specific other categories per TRICARE rules
Each beneficiary's eligibility is independent.
Common TFL Questions
"Do I need to enroll in Medicare to get TFL?"
Yes. Medicare Part A and B enrollment is required for TFL eligibility. Enroll through Social Security 3 months before age 65.
"What does Medicare Part B cost?"
Monthly premium set annually by Medicare. Standard premium rises with income (Income-Related Monthly Adjustment Amount or IRMAA).
"Will TFL cover costs Medicare denies?"
If TRICARE covers the service, TFL pays the cost Medicare denied. If neither program covers the service, beneficiary pays.
"I see any doctor with TFL?"
You see any provider who accepts Medicare. The provider does not need to be in the TRICARE network.
"What about prescription drugs?"
Medicare Part D is not required (and might complicate TRICARE pharmacy coverage). TRICARE pharmacy benefits cover drugs through.
- Military treatment facility pharmacies (no copay)
- TRICARE Mail Order Pharmacy (low copay)
- Retail network pharmacies (higher copay)
Common I&I Errors on TFL
These hit at the I&I shop.
- Telling member they need to enroll separately for TFL. No separate enrollment beyond Medicare. Coverage automatic with Medicare enrollment and DEERS retiree status.
- Confusing TFL with Medigap. TFL is the military equivalent of Medigap but operates differently. Provided through DOD, not commercial insurer.
- Suggesting Medicare Part D. Most TFL beneficiaries do not need Part D since TRICARE pharmacy is comprehensive. Adding Part D might complicate coordination.
- Wrong age for Medicare. Standard Medicare eligibility age is 65. Disability-based Medicare might start earlier.
- DEERS not updated. Retiree must verify DEERS shows Medicare enrollment. Without DEERS update, TFL might not coordinate with Medicare claims.
