Medical Board Process - MEB and PEB
Members with permanent or potentially permanent medical conditions undergo the Disability Evaluation System (DES). The Medical Evaluation Board (MEB) reviews the condition. The Physical Evaluation Board (PEB) determines fitness for duty and disposition.

What the Medical Board Process Is
Per DoDI 1332.45DoDI 1332.45 and MCRAMM Chapter 6, the Disability Evaluation System (DES) determines fitness for duty for members with conditions affecting service. The system has multiple stages.
Stage 1 - Medical Evaluation Board (MEB)
The MEB reviews the member's medical condition.
MEB Purpose
- Document the member's medical condition
- Determine if condition affects duty
- Identify whether disposition is needed
- Recommend referral to PEB
MEB Process
- Medical provider identifies condition affecting duty
- MEB convened to review
- MEB reviews medical records
- Member might submit additional documentation
- MEB determines findings
- Member receives MEB findings
- Member might request review or accept findings
MEB Findings
Possible findings.
- Member fit for full duty (no further action)
- Condition does not warrant referral to PEB
- Recommend referral to PEB for fitness determination
Member's Rights at MEB
- Review of MEB findings
- Submit comments
- Request review
- Submit additional medical documentation
Stage 2 - Physical Evaluation Board (PEB)
If MEB recommends, member proceeds to PEB.
PEB Purpose
- Determine member's fitness for duty
- If unfit, determine disability rating
- Recommend disposition
PEB Findings
Possible PEB findings.
Fit for Duty
- Member returns to full duty
- No disability rating
- Return to drilling status
Unfit - Disability Rating Below 30%
- Member separated with disability severance pay
- Specific separation processing
- VA might rate separately
Unfit - Disability Rating 30% or Higher
- Member retired with disability retirement
- Continuing pay (percentage of basic pay)
- TRICARE eligibility continues
- Specific retirement processing
Unfit - Temporary Disability Retirement (TDRL)
- Member temporarily retired
- 5-year maximum on TDRL
- Periodic re-evaluation
- Final disposition after re-evaluation
Integrated Disability Evaluation System (IDES)
IDES integrates DOD and VA processes.
IDES Purpose
- Single set of medical exams
- DOD and VA disability determinations made together
- Streamlined transition for separated members
IDES Process
- MEB completes
- VA conducts disability examinations during DES
- VA provides disability ratings
- PEB uses VA ratings for fitness determination
- Member receives both DOD and VA disability decisions
IDES Benefits
- Faster overall process
- Single set of exams (rather than separate DOD and VA)
- Better coordinated care
Reserve Member DES
Reserve members have specific DES considerations.
Reserve LOD Determination
Per MCRAMM Chapter 6.
- LOD determination drives DES eligibility
- LOD-positive condition qualifies for DES
- LOD-negative does not
Reserve DES Pay Status
- Might continue drilling status during DES
- Or might be on medical hold (active duty status)
- Specific situation drives pay
Reserve DES Disposition
- Same disposition options as active duty
- Specific Reserve component considerations
- Coordinate with Reserve component
Performance Steps for I&I Admin
Step 1 - Coordinate MEB Initiation
When member's condition triggers MEB.
- Medical provider recommendation
- Member counseling about MEB
- Coordinate with MEB processing
Step 2 - Support Member During MEB
Provide assistance.
- Documentation gathering
- Member's rights briefing
- Specific procedural support
Step 3 - Process MEB Findings
After MEB.
- Document findings in member's record
- Coordinate next steps
- Specific actions per findings
Step 4 - Coordinate PEB if Required
If MEB recommends PEB.
- PEB scheduling coordination
- Continue documentation support
- Member counseling
Step 5 - Process PEB Findings
After PEB.
- Document findings
- Coordinate disposition
- Specific actions per findings (return to duty, separation, retirement)
Step 6 - Coordinate Disposition
Per PEB findings.
- Return to duty: UD/MIPS transaction
- Separation: Disability severance processing
- Retirement: Disability retirement processing
Common DES Issues
Long DES Process
DES take many months.
- Coordinate ongoing treatment
- Member status management during DES
- Pay implications
Member Disagreement with Findings
Member might disagree with MEB or PEB findings.
- Specific appeal procedures
- Member's right to review
- Coordinate appeal support
LOD Issues for Reserve Members
LOD determination critical for reserve DES.
- Coordinate LOD investigation
- Verify LOD-positive for DES eligibility
- Specific Reserve provisions
DES vs Standard Separation
DES is medical-driven.
- Different from standard administrative separation
- Different timelines
- Different benefit implications
DES and Pay
Pay during DES.
Active Duty Member in DES
- Continues active duty pay
- BAH and BAS continue
- TRICARE Prime continues
Reserve Member in DES
- Drilling status might continue with restrictions
- Or active duty if Medical Hold
- Specific situation drives pay
Disposition Pay Impact
- Return to duty: Pay continues normally
- Disability separation: Severance pay (covered separately)
- Disability retirement: Continuing pay at percentage of basic pay
- TDRL: Continuing pay during temporary status
Documentation Required
For DES processing.
- Medical evaluation supporting referral
- MEB findings document
- PEB findings document
- Member's responses and submissions
- Disposition documentation
Coordination Touchpoints
DES processing touches.
- MTF Medical Provider: For medical evaluation
- MEB: For initial board review
- PEB: For fitness determination
- VA: For IDES coordination
- Member's chain of command: For disposition coordination
Quality Control on DES
Common errors.
- Documentation incomplete. DES requires comprehensive medical documentation.
- Member rights not communicated. Member should know rights at each stage.
- LOD coordination missed. Reserve DES depends on LOD finding.
- Disposition follow-through delayed. Final disposition after PEB requires action.
