Periodic Health Assessment (PHA)
The PHA is the annual medical screening for all military members. Reserve members complete PHA annually to maintain readiness category. The PHA covers physical exam, mental health screening, dental, and specific deployment-related items.

What the PHA Is
Per DODI 6025.19DODI 6025.19 and MCRAMM Chapter 6, the Periodic Health Assessment (PHA) is the annual medical screening for all military members including Reserve. The PHA establishes baseline health and identifies any conditions affecting readiness.
PHA Components
The PHA includes.
Self-Reported Medical History
Member completes questionnaire covering.
- Current medications
- Past medical history
- Current symptoms
- Mental health status
- Specific deployment-related concerns
Physical Examination
Health care provider examines member.
- Vital signs
- General physical examination
- Specific deployment-relevant evaluation
Specific Tests
As indicated.
- Vision screening
- Hearing screening

Mental Health Screening
Standardized mental health screening.

Dental Status
